Reporting an incident online

Reporting an incident online

The council has in place a system for reporting accidents and other health and safety related incidents. The online system, and relevant instructions, should be used to record any incidents. Information on this approach can be downloaded from this page.

Two different guidance manuals are available. The first is for those that are creating a new record, referred to as the "Inputter". This can be the injured/ affected employee, their manager or another employee acting on behalf of the affected person. Input Guide

The second guidance manual is intended for use by the Manager/ Supervisor and aims to help them complete the summary of investigation section. You should use the one that best suits the role you are looking to fulfil. Manager Guide to Recording Investigations on OSERT

Once an accident/ incident has been recorded, managers will receive an email to approve the record.  Once the Service investigation process has been completed the manager can then approve the incident (normaly withi 10 days) and have it submitted to the Health and Safety team for review.  Once agreed by the team an "incident summary" email will be issued to the manager and the "inputter" and this can be used as the accident book entry.

Summary steps

Inputter/ Affected person inputs basic information onto the database via People Connect "Employee Menu".

An alert email will advise the relevant manager an incident has been recorded and requires actioning.

The relevant manager will investigate and update the record, accessed via People Connect Manager, before moving the record to the Health and Safety Team for actioning.  The manager will alert the Health and Safety team by email if the incident has resulted in an absence of greater than 7 days, or if the person has not been able to undertake their normal duties for more than 7 days.

The Health and Safety Team will evaluate the content and undertake actions needed to ensure reporting to the appropriate enforcement authorities takes place.

The Health and Safety Team will approve the record for upload to the relevant personnel record. This will generate an email summary that is sent to the inputter, an email summary will also be sent to the manager to enable a local accident book to be maintained.

Categories of people that accidents/ incidents should be reported for

There are a range of obligations placed upon the council depending on who has been involved in the incident. For that reason those creating a report will be asked to indicate what category of person the affected indiviual is to be classed as. These classifications are as follows:

Category of person Description
Client / Service user Those who are in residential care, make use of day care services or are visiting Housing, Social Work or Q&A offices.
Contractor Someone employed by another organsiation but undertaking work tasks that have been commissioned by the Council.
Employee Someone who is employed by the Council.
Member of the public This is someone who is visiting/using a Council property but not in any of the other roles outlined here.
Person on work experience This would be a person who has been placed within a Council service on work experience. This is often someone who is school work experience but could also be someone placed by a university/college.
School pupil This is someone who has been involved in an incident whilst they are considered a school pupil, ie during school hours or whilst on excursions/trips organised by the school.
Self employed contractor This would be someone who has been given a contract to undertake certain duties for the Council and are the sole employee of their organisation.
