Admission to long-term care

Admission to long-term care

This page gives practitioners information on internal processes as well as national resources, in relation to facilitating an individual moving from the community to a care setting.

An SDS Briefing "From SDS to long-term care" can be viewed through the SDS Briefings page.

Emergency respite

In some circumstances, a supported person may be admitted to respite on an emergency basis, and then considered for long-term care.

Once a Fieldwork Manager approves emergency respite, an emergency respite record requires to be added to supported persons D11 for two-week period, while the Living the Life You Choose Assessment is being completed: this prevents delays in payment of invoices. If the assessment process takes longer than two weeks, a further record requires to be added to D11.

The relevant Operations Manager will confirm the date that long-term care funding has been approved, the D11 emergency respite record requires to be ended on the same day long-term care funding commences.

Considering potential care placements

Age Scotland provide good resources for the supported person, family and even workers when having discussions over long-term care:

Care Homes information and advice

Care Home Guide Funding

Age Scotland has also developed a helpful checklist which loved ones and families can use when considering a placement.

Care Information Scotland provide the most up-to-date rates and financial advice for families considering long-term care.

We also recommend families look on the Care Inspectorate website to review inspection reports prior to any placements.

SLC workers can find further information on care settings on our Residential Care Homes page.

Reviews

Long-term care placement reviews should be conducted at regular intervals to ensure the wellbeing and quality of care for residents. Guidance has been developed in relation to purpose, frequency, governance and key areas to be considered.

Please refer to the Long-term Placement Review Guidance.
